1. Join a Zoom blitz/Demo/Trial as a Patient (subsidized/cheaper) or Non-Patient (more $$, no health info required)
2. Purchase a membership or program on the MedResRx Hypnotherapy App (Founding Membership is currently on and the best deal)
3. Request for Individual Coaching/Hypnotherapy/Psychotherapy with Dr. Denise (currently a wait list).

1. MedResRx Patients: Manitoba/Canadian residents with current valid health card may access subsidized therapy (discounts) with Dr. Denise:
   1. You must review/sign/send to drdenise@medresrx.com the following forms:
      1. Completed Therapy Intake Form including:
      •  full name/email/address
      •  valid Canadian health information number,
      •  date of birth,
      •  sex, and
      •  diagnosis/issue
      1. Consent to e-Communications
      2. Hypnosis Disclosure Form
2. Non-Canadians/Non-Patients of Dr. Denise:
   1. You can access sessions using the Non-Patient links provided.
